WIN/LOSS REQUEST FORM

You may obtain your Win/Loss information immediately by clicking the following link: Fantasy Rewards Login. You will need your Players Club Card account number and password. If unable to login to your account, please contact our Guest Services staff at (800) 827-2946 or (760) 342-5000.

 

Any information provided on the form must match that of the requestor's FSRC Players' Club account in order for a statement to be prepared. The Win/Loss Statement(s) will reflect our records of card-in play for the year(s) requested as of the date indicated on the form. It is an individual player's own responsibility to keep accurate records of gaming wins and losses for income tax purposes each tax year. The Statement will only provide an estimate of wins and losses and is dependent on the proper use of the Players Club Card. Due to the high volume of requests during tax season, please allow 2 – 3 weeks for the processing of your request. The security of your personal information is of utmost important to us. You should be aware that there is always some risk involved in transmitting information over the Internet. We follow generally accepted industry standards to help protect your personal information. We employ reasonable security measures to protect the security of the information. No method of transmission over the Internet, or method of electronic storage, is 100% secure. Peter Frampton

Peter Frampton is a British rock musician, singer, songwriter, producer, and guitarist. He was previously associated with the bands Humble Pie and The Herd. At the end of his 'group' career was Frampton's international breakthrough album, his live release Frampton Comes Alive!. The album sold more than 8 million copies in the United States and spawned several hit singles. Since then he has released several major albums. He has also worked with David Bowie and both Matt Cameron and Mike McCready from Pearl Jam, among others.
